HMO (Health Maintenance Organization)
A managed-care plan that covers in-network care and usually requires a primary care physician referral.
HMOs emphasize preventive care and lower costs in exchange for a restricted provider network. Members typically choose a primary care physician (PCP) who coordinates care and refers to specialists. Out-of-network care is generally not covered except emergencies.
